Urgent Medtronic Neuromodulation INDURA IP Product Recall
October 2, 2008
FDA has issued a Class I Recall on Medtronic Neuromodulation INDURA IP intrathecal catheters, sutureless pump connector revision kits and intrathecal catheter pump segment revision kits.
There are potential misconnections of the Medtronic sutureless connector (“SC”) catheters from the catheter port on the medtronic pump. These misconnections have resulted in a blockage (occlusion) between the sutureless pump connector and the catheter port on the pump and disconnection from the pump connector.

Nebraska Beef Recalls 1.2 Millions Pounds of Beef for Possible E. Coli Contamination

Nebraska Beef, Ltd., an Omaha, Neb., establishment is recalling approximately 1.2 million pounds of primal cuts, subprimal cuts and boxed beef that may be contaminated with E. coli O157:H7, the U.S. Department of Agriculture’s Food Safety and Inspection Service announced today.

Chanitx and Suicide

More reports of erratic behavior and suicide have been associated with the use of Chantix, a drug that was designed to help people stop smoking and was approved by the Food and Drug Administration just under two years ago. In November 2007, the FDA issued a statement that they are reviewing the possible Chantix suicide side effects and that there have been over 100 reports received of suicidal thoughts and severe behavioral changes within just a few weeks of taking the drug.
143 Million Pounds of Beef Recalled by USDA in Largest Beef Recall in History

The United States Department of Agriculture (USDA) today ordered the recall of 143 million pounds of frozen beef – the largest beef recall in US history.
The recalled beef cam from a Chino, California slaughterhouse called the Westland Hallmark Meat Company. This slaughterhouse is the subject of an animal abuse investigation and provided meat to schools across the country.

Evenflo Announces Recall/Revision of Discovery Infant Car Seat

On February 1, Evenflo announced a voluntary recall/revision of its Discovery infant car seats, Models 390, 391, 534, and 552. Recent laboratory tests conducted by Evenflo and the National Highway Traffic Safety Administration (NHTSA) showed that the seats could become detached from their bases in high impact side collisions.
Heparin Syringes Recalled

A nationwide recall has been announced on Pre-filled Heparin Lock Flush Solution USP (5 mL in 12 mL syringes) with Lot #070926H. The Heparin IV flush syringes have been found to be contaminated with Serratia marcescens, a bacterium that is resulting in patient infections. This type of bacterial infection can cause severe, life threatening or life altering injuries and can even be fatal.
Mattel Announces Toy Recall

On November 6, 2007, Mattel Inc. recalled over 172,000 Fisher-Price kitchen toys because several children have choked and gagged on small, detachable parts. Mattel has received approximately 50 reports of small parts separating from these toys, which feature a sink, a refrigerator, and a range. One child choked on a detached piece and needed the Heimlich Maneuver performed on him to remove the part that was lodged in his throat.

Fairbank Farms is a DBA of Fairbank Reconstruction Corp., who discovered the contamination danger during their routine microbiological testing program.
Life Saving Recall for Scuba Divers

M&J Engineering has Recalled about 1,500 swivels for scuba diving masks due to drowning hazard, as the swivel could separate while diving.
This M&J Engineering recall includes all SW-3000 2nd stage 360-degree swivels for scuba diving masks manufactured between January 2004 and February 2005. The swivel has the manufacture date stamped on the body, below the patent number. Swivels that are already repaired have an arrow stamped on the top right corner of the swivel body.

Millions of Ford Cars and Trucks Recalled for Cruise n Burn Setting

After years of reported cases of vehicles catching fire, Ford has recalled 3.6 cars, SUVs, Mini Vans and Trucks due to problems with the speed control deactivation switch. According to Ford’s website:
“Ford is taking this action to address continued customer concerns about the potential for fires in their vehicles. We cannot be confident in the long term durability of the speed control deactivation switches.”
